Utility Activation Forms

You will find the Utility Activation Form (UAF) broken out by the state in which the property is located. You will also notice a date range. Please select the appropriate form based on the date when the utilities are to be activated. If no date range is indicated, such as in the state of Alaska, there is only that one form regardless of the time of year.

Washington - UAF

Oregon - UAF

California - UAF 

Hawaii - UAF

NOTE:

Alaska: Requires year round winterization.

Hawaii: Does not require winterization.

Washington: Winterize from 9/15 through 4/15.

Oregon: Winterize from 10/1 through 3/31.

California: Winterize from 10/1 through 3/31 when elevation above 2000 ft. Please contact the regional office for clarification of elevation.
