Vendor Corner



Please do not contact anyone in our regional offices requesting information on vendor opportunities. Please contact Dan Hanson should you have specific questions regarding available opportunities. Thank you for your anticipated cooperation.

Please note we are still seeking additional vendors for all other contract areas.

Here are the 5 Simple Steps to becoming an approved vendor of Cityside Management.

Step 1. Complete the Contractor Application (if you are experiencing issues viewing this link, please right click on the link and select "Save Target As". This will allow the application to be downloaded. Select your destination and click "Save". Once the download is complete, select "Open". Adobe Reader is required to view a PDF document. Step 2. Complete the Request for Taxpayer Identification Number (W9) 

1.  Only one box can be checked under the federal tax classification.
2.  An LLC is not always the same thing as a partnership.
     A partnership can be a corporation.
3.  New classification: Trust/estate. This will require the trust/estate paperwork and it may have either an EIN or a social
     security number depending on the type of trust/estate.
4.  Exempt payee: this will rarely be valid if it is checked off.
     Documents from the IRS will be required.
5.  LLC: there needs to be the tax classification entered with
     this. C, S, or P.

Step 3. Obtain a Certificate of General Liability Insurance. Minimum limits for General Liability are $1,000,000 per occurrence and $2,000,000 aggregate coverage.

Step 4. Include your company name and signature of principal contact of your company on the Sub-Contractor Agreement.

Step 5. Once the above steps are complete you will be emailed a link to view our Training Video. You will be required to submit a digital signature and acknowledge that you’ve participated in our online training. You will also be given access to Cityside’s Lockbox & Key Codes, which can be found on the same link.
